Safe and Well- Inquiry Form

This is used when someone is seeking information on a person or organization that may have been affected by disaster.
It is text format to allow it to be copied and pasted in the body of a Winlink message
Safe and Well-Outgoing Form

This form is used to report on a displaced person or a person affected by disaster.  This form when completed should be sent to a Winlink station that still has a functioning Internet connection.  This should be copied and pasted into the body of a Winlink message

ITU Phonetic Alphabet

This is Word document with a listing of the phonetical alphabet.  Unless you have a good grasp of this from memory, print a copy and take it with you on assignment.
ICS-309 Radio Log

This is the standard ICS logging document for radio communications.  This is in a pdf format that will require writing in the information.
ICS-213 General Message Form-TXT

This form is in a text format so that it may be copied and pasted into a Winlink message body.  This allows for a more rapid sending rate than an attachment.
ICS-213 Fillable Word Version

This form is ideal when working with a computer and printer.  This should be used for voice transmission.  It may be printed as a blank and filled in by pencil or pen.
ICS-205 Frequency Assignment

This will be completed when repeater permission is secured,  This will become our frequency plan for the exercise
This link is used by stations outside the impact area and are transferring incoming Winlink forms to the simulated database.  Stations using this must have a connection to the Internet. (Simulation)
This link is used by stations or individuals outside the impact area that have an Internet connection.  They are filling out these forms when looking for friends or family that have been impacted by the disaster.  (Simulation)

A description of the items that describe a resource that is being requested from the Virignia Department of Emergency Management.  Use this when sending an ICS-213 form to VDEM.
